A housing consultant helps individuals who do not have Medical Assistance (MA) case management by developing a personalized, person-centered plan. This plan focuses on identifying their needs, wants, and long-term goals for living independently in the community.
How will we help?
- Assess the Client's Needs: Evaluate the client’s housing situation, including their goals, preferences, and any challenges they face in securing housing.
- Develop a housing focused person- centered plan: Create a tailored, person-centered housing plan that addresses the client’s specific needs, wants, and goals.
- Provider Choice: helping a person make an informed choice in their housing transition or sustaining services provider.
- Guidance: Offering resources related to non-housing goals; and coordinating with other service providers already working with the person.
This role focuses on providing comprehensive support for individuals looking to secure and maintain stable housing.
